Benefits Open Enrollment
Open Enrollment Dates: November 13-December 8, 2008
IMPORTANT NOTE: All forms must be received by the Human Resources department on or before December 8, 2008 in order for benefits to go into effect for the 2009 plan year. There will be no exceptions. Failure to adhere to this deadline may jeopardize the College's plan status under IRS Section 125.
Open Enrollment for Concordia College employees begins on Thursday, November 13, 2008 and continues through Monday, December 8, 2008. This is an opportunity to review your insurance benefits and determine what coverage you would like for the next year. It may also be a good time to review beneficiary designations for Life and Retirement. We have added some options to our dental and life insurance plans so please be sure to review them.
Forms are available from the Human Resources department, at the informational meetings, and at Benefit Forms on our website. Once the forms are completed, please sign them and submit "hard-copy" originals to the Human Resources department. Moorhead and St. Paul staff will turn their paperwork in to Lynn Dixon at Moorhead; Bemidji employees will turn them in to Joanne Barfknecht at Bemidji.
If you would like to maintain your current coverage and do not plan to make any changes for the upcoming year, then you do not need to take any action during Open Enrollment. Please note, however, that if you wish to participate in a Flexible Spending Account, you would need to re-enroll for 2009 otherwise, you will NOT be participating in 2009.
Informational meetings will be offered by David Lysne from Benefits for Success to review the Life Insurance and Long Term Disability (LTD) benefits on Thursday,11/20 (at 1:00 and 2:00 p.m.), Friday, 11/21 (at 10:00 a.m. and 3:00 p.m.) and Tuesday, 12/2 (at 9:00 a.m.) in the Frida Nilsen Lounge at Fjelstad Hall. Sessions will be available via videoconferencing at Bemidji and St. Paul. David will also be available for drop-in individual meetings in the HR Conference Room on 11/20 (2:30 - 3:30 p.m.), 11/21 (10:30-11:30 a.m.) and 12/2 (9:30-10:30) or you may call him directly at 701-365-8780. The Human Resources department will be on hand during these times to meet individually to answer benefit questions and provide paperwork for open enrollment.
