Reminder: Please complete this form BEFORE you begin the Handshake process through the Career Center. Your Handshake experience request will not be reviewed until you have first completed this CPT Information Form through Global Learning/International Community.

Before you begin: If you plan to use a PEAK for this CPT experience, make sure you have reviewed important information about using a PEAK when registering for CPT. Because only two PEAKs at Concordia count toward graduation, only the first two PEAKs can be used for CPT registration.
