This form is available to members of the Concordia College community who have either experienced or witnessed sexual assault or another form of sexual harassment. All information provided will be forwarded to the Title IX coordinator. The information will be used to track reports of sexual violence for reporting purposes to state and federal authorities. This information will also be used by the college to better understand the incidence of sexual violence in our community, and to aid Concordia in its education, prevention, and response efforts. Providing information on this form will not initiate Concordia's grievance process.

Individuals who have experienced sexual assault or another form of sexual harassment are encouraged to make a report. A report does not mean that a complaint must be pursued. To make a report the following may be contacted:

It is important to understand that there are resources and services available to you.

The new regulations require us to post the training that members of the Title IX team have taken. We are trying to identify if we need to simply post the title and date or if it must post the entire training program.
