At Concordia College, your safety and well-being is our primary concern. Whether you are reporting a concern, submitting a witness report, or are unsure of your rights or what actions can be taken for an incident that has given you cause for concern, Concordia College provides resources and information for you to make decisions about your experience.

Concordia College provides students and employees with an online system for easy and confidential reporting of concerns, violations of the Student Conduct Code, Title IX, Discrimination and Bias, and more. Please review the reporting forms below to select the reporting form that best meets your needs. If you are unsure of which form to choose, please complete the Campus Culture Concern reporting form.

Title IX

Report any action or behaviors that are sexual in nature or discriminatory because of a person’s gender. 

Security and Public Safety
Report any non-emergency incidents that relate to campus safety, public safety, or security on campus.

Risk Management Incident Report
Report misconduct for incidents or procedures that pose a risk to the assets of the college.

Whistleblower 
Anonymously report any misconduct, policy violations, unethical behaviors, or illegal activity happening within the institution. This report is managed through an outside investigation agency.

Residence Life Care Team Report
Report a student who may need support for their emotional well-being, meeting their basic needs, alcohol or drug use, physical well-being, mental health, or academic progress.

Center for Student Success Academic Concerns
Report a student for attendance concerns, risk of failing a course, or other academic barriers to success in a course.

Student Complaint
Relating to Concordia’s Student Complaint and Resolution Policy, report a complaint about institutional misconduct of its policies, safety risks, mishandling of course expertise, or falsely representing the quality of programs offered and violating state or federal laws.

Campus Culture Concern
Report an incident that includes activity, behaviors, or communications that intimidates, demeans, mocks, degrades, stereotypes, marginalizes, or threatens individuals or groups based on that individual’s or group’s actual or perceived age, ancestry, ethnicity, national origin, ability, sex, gender identity or expression, citizenship or immigration status, marital status, socio-economic class, race, religion, religious practice, sexual identity, or veteran status.
